Responsibilities
- Execute telephonic outreach and scheduled video or phone appointments for recently discharged hospital patients
- Perform medication reconciliation, conduct symptom screening, and order necessary follow-up laboratories, medications, or supplies
- Establish trust with patients, explain the organizational care model, and link individuals to appropriate health services
- Coordinate care transitions between hospitals, emergency departments, pharmacies, and community providers
- Partner closely with the nursing department and adhere to individual performance metrics alongside team population health goals
Requirements
- Active state medical license in Connecticut
- Availability to commit to a minimum of 24 hours per week
- Certified Nurse Practitioner in Adult, Family, or Geriatrics
- Minimum of five years of post-graduate clinical background in Primary Care or Geriatrics, though two years may be fulfilled via experience in a skilled nursing or long-term care facility
- Direct experience engaging Medicare and Medicaid patient groups with complex and comorbid conditions
- Proficiency in motivational interviewing techniques
- Familiarity with virtual care delivery and standard digital tools such as Google Suite and Slack
Qualifications
- Active state licensures in New Jersey, Texas, Pennsylvania, Illinois, New York, Massachusetts, Minnesota, Louisiana, and or South Carolina
- Bilingual communication capabilities in English and Spanish with associated cultural fluency
